Community Paramedicine Remote Patient Monitoring
Empowering Health Through Innovation
Established in 2011, our organization was founded by the South Central Community Development Corporation (SCCDC) with the primary purpose of providing home monitoring services to patients with chronic conditions across Canada. In 2012, the SCCDC initiated a pilot project to collaborate with municipalities to explore how technology could help seniors live safely in their homes for as long as possible. The result of the pilot project led to the creation of a social enterprise known as Future Health Services (FHS), which received funding from Canada Health Infoway to develop a cost-effective, sustainable, and scalable business model aimed at reducing unnecessary healthcare resource utilization among frequently visiting patients. FHS called the program the Community Paramedic Remote Patient Monitoring program (CPRPM) and selected Queens University to evaluate the program
CPRPM’s Measurable Success
Queens University conducted an independent review of the program and found that for the patients in the study:
- A 31% reduction in the number of EMS 911 calls, with more complex patients experiencing even greater decrease of 47%.
- The number of visits to Hospital Emergency Departments decreased by 31%.
- The number of Hospital Admissions dropped by 40%.
- There was a 41% reduction in 30-day Hospital Re-Admissions.
- The average Length of Stay in hospitals was reduced by 22%.
- Resulting in a 510% Return on Investment to the Health Care system.
FHS’s National Reach and Vision
Future Health Services’ vision is to provide remote home monitoring services to patients with chronic conditions, aiming to empower and educate individuals across Canada. We are dedicated to improving patient care within the home and reduce distance barriers through innovative home monitoring solutions.
Our goal is to revolutionize healthcare by empowering patients and providers with seamless, real-time remote monitoring solutions that improve health outcomes, enhance quality of life, and promote proactive, personalized care for individuals everywhere. Since inception, the program has expanded and FHS has monitored over 5,000 patients in 40 of the 52 municipalities in Ontario and Prince Edward Island.
