About the Program

The CPRPM (Community Paramedicine Remote Patient Monitoring) program is an initiative led by Community Paramedics that has been designed to assist patients with chronic illnesses such as congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD), Diabetes and many others. Designed to provide continuous oversight and early intervention, the program’s core mission is to help patients remain safe, stable, and comfortable in their own homes- while reducing the need for emergency department visits and hospital admissions.

Remote Patient Monitoring

Remote Patient Monitoring (RPM) is a key component of our telehome monitoring services. It allows patients to be monitored from the comfort of their homes, with vital health information shared with their care providers to ensure continuous and effective care.
Remote patient monitoring (RPM) is a straightforward concept. Through the use of modern technology, Paramedics can collect essential health data such as weight, blood pressure, oxygen levels, and temperature from patients in their own homes on a daily basis. Healthcare providers can track these vital signs from virtually anywhere and identify trends that could indicate a patient’s condition is deteriorating.

By continuously monitoring patients in this way, the healthcare team can step in early to adjust medications, change lifestyle habits, or arrange for an in-person visit. Research consistently shows that this proactive approach leads to fewer emergency room visits, reduced hospital admissions, longer periods spent at home, and greater patient control over their health.
