Community Paramedicine Remote Patient Monitoring
About the Program
The CPRPM (Community Paramedicine Remote Patient Monitoring) program is an initiative by Community Paramedics that has been designed to assist patients with chronic illnesses such as congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD), Diabetes and many others. The program’s primary goal is to keep these patients safe at home and reduce the necessity for hospital visits.
Objectives
- Improve Quality of Life: Enhance patients' daily living conditions.
- Improve Communication: Foster better coordination among healthcare providers to deliver high-quality care.
Partnership
The CPRPM program is conducted in partnership with Future Health Services and Community Paramedics, leveraging their advanced telehome monitoring products to provide effective and efficient patient care.
Implementation
Device Setup
- Patients are identified as candidates by paramedics or healthcare professionals.
- Community Paramedics set up homes with user-friendly devices.
Devices Provided
Depending on the health condition, patients may receive:
- Weight scale
- Blood pressure monitor
- Heart rate monitor
- Glucometer
- A Gateway that connects these devices to paramedics in real time.
Monitoring Process
- Daily Use: Patients use these devices daily to record their biometric data.
- Alerts: If readings go outside set thresholds, an alert is generated to Community Paramedics.
Follow-Up
- Community Paramedics will call the patient to review their health status if an alert is triggered.

How It Works
- The remote monitoring program builds on municipal Community Paramedic initiatives by adding remote monitoring capabilities, allowing paramedics to monitor patients with the same human resources.
- Paramedics set up the monitoring equipment in the patient’s location and train the patient to use it.
- Patients take daily readings are taken from the patient’s location and are remotely sent to the paramedic portal for the paramedic to view.
